I will go first.

Last night I was changing my injectors on my car at a friend's house (who had a good extra set).  I was carefully alternating tightening each bolt until I got to the last one and was using my 'calibrated' elbow to tighten the last one and POP!  "D**N IT!"  Start looking and noticed a whole chunk of intake manifold was loose now.....I snapped off a corner chunk of manifold and now have a hole the size of a dime in the side of a runner.  We epoxy the chunk back in and let it dry for about 1 1/2 hours.  We get everything bolted back up and start up my car.  It runs but idles like a diesel truck.  I retard my timing a few degrees, load up and go home.  I will be heading to the junkyard today for a new manifold, rain or snow.

I've got two!

First of all, I was installing an AutoMeter oil pressure gauge in my Duster.  I was torquing the adapter into the hole, and apparently there was a defect in the adapter.  The hex head broke off, leaving the "hollow bolt" stuck in the cylinder head.  And that car was my daily driver at the time...

My second story is changing the cone air filter on the turbocharger.  I installed that air filter when the motor was out of the car during the engine swap.  The screw on the ring gear was facing the rear of the motor, where I could absolutely NOT get to it when the motor was in.  So I had to use a BFH and a long screw driver to break the ring gear clamp and rip the original air filter in two in order to get it out!

Just did one, installing a DP onto an evo 3 that someone else had made. The holes on the face of the DP weren't drilled very precise as i found out. Was pulling it into position with a bolt and it started to feel a bit tight. Hmm better take that out...TOO LATE...SNAP. Had to that the turbo off and get the machine shop to take it out. afternoon work turned into 4 days with rain delays...

Sure..... back in my early turbo days... didn't check the flow of the oil feed for the turbo before installing the turbo and going for a short drive.  By short drive, I mean less than a mile.  Pull back in and hear a little tick tick tick.  Argh.... damaged the journal bearings   Out came the turbo and off for a rebuild!

The first time I went to put in some coil overs on my NX. I set the ride height 1in lower than it was sitting before the swap. Then went to NTB to get the car aligned. They took the car no problems even after I told them I just put on some new coil overs and left thinking they must do this stuff all the time, because the other alignment places I took the car to before hand turned me away. I let them know that the holes on the strut bodies were slotted and they said they understood.

An hour after I dropped the car off they called back and said they couldn't get the alignment right and asked if the car had been in a crash. I told them it defiantly hadn't been in a crash, so they told me I needed some camber/crash bolts for like $100 per side but only two sides needed it. I reluctantly agreed with no other places in town wanting to touch my car since it was modified (and I didn't know how to align the car myself at the time) Two hours after that they called back and said they still couldn't get it right but they had another idea. I was just like whatever just do it. They said it would be done in an hour and they would call me when it was done. Three hours later they called and said it was done and  I went down to pick my car up.

They told me about how they had never seen coilovers before, but I got lucky that one of the guys that works there had a drill in his car. I didn't ask I was just happy to have my car back before work. I paid something like $450 after the discount they gave me and left. On the drive home the car swerved and thumped around a corner. I stopped and looked around to see what the problem was and the  bolts on the strut were loose on two sides. I had forgotten to tighten them down before I went to NTB thus making the alignment constantly change and confusing the hell out of the NTB idiots

And the best part was about a year later when I found out what they used the drill for. They drilled out my cars strut towers and put some thin little washers to make some horribly dangerous ghetto camber plates
